If you are receiving a breast thermogram, please download the forms in this section and complete them before coming in for your appointment if possible.  If you are new to our clinic. Please complete the forms found under Patient Registration below.  Pay particular attention to the information on what not to do prior to your thermogram.  Following these instructions is necessary for an accurate thermogram

Full Body / Region of Interest Thermography
